              •   Grouping “A” (Price Lines 00001 – 00003): adult nutritional formulas; specifically 1.0, 1.5 and 2.0 calorie oral formulas 
                  In 8 oz resealable or brik pak containers only.  
                  (Other packaging/sizes/varieties of adult nutritional formulas, including 1.0, 1.5 & 2.0 cal are in Grouping “I” Price Lists) 
                   
              •   Grouping “B” (Price Line 00004 only): pediatric 1.0 calorie oral nutritional formula.  
                  In 8 oz resealable or brik pak containers only.  
                  (Other packaging/sizes/varieties of pediatric oral nutritional formulas are in Grouping “I” Price Lists) 
                   
              •   Grouping “C” (Price Line 00005 only): 1.0 calorie adult oral glucose tolerance formula. 
                  In 8 oz resealable or brik pak containers only.  
                  (Other packaging/sizes/varieties of glucose tolerance formulas are in Grouping “I” Price Lists) 
                   
              •   Grouping “D” (Price Lines 00006 – 00008): 5 oz pudding-thick oral formulas. 
               
              •   Grouping “E” (Price Lines 00009 – 00011): 4 oz pudding-thick oral formulas. 
               
              •   Grouping “F” (Price Line 00012): 1.2 calorie high nitrogen density tube formula with the salient characteristics as specified.  
                  (Alternatives are in Grouping “I” Price Lists) 
                   
              •   Grouping “G” (Price Line 00013): 1.0 calorie isotonic fiber rich tube formula with the salient characteristics as specified. 
                     (Alternatives are in Grouping “I” Price Lists) 
          
              •  Grouping “H” (Price Line 00014): phe-restricted therapeutic dietary products.  These have been added as an independent line item 
                  requiring an EXCLUSIVE N.J. NET PRICE LIST. 
               
              •  Grouping “I” (Price Line 00015 only): requires an EXCLUSIVE N.J. NET PRICE LIST to be provided for items that are different nutrition 
                  formulas or packaging types than as required in Groups “A”-“G”.  This includes flavorings, additive and alternatives to the  “A”-“G” products. 
          
              •   The administration sets (operational): 
                  Grouping “J “(Price Line 00016): tied to Conventional feeding-pump equipment or 
                  Grouping “K” (Price Line 00017): tied to Self-flushing feeding-pump equipment.   
                  Both Grouping “J” – Conventional and Grouping “K”- Self-Flushing require EXCLUSIVE N.J. NET PRICE LISTS to be attached.  
                    
              •   Non-operational administration sets and/or feeding-pumps and related equipment and other interchangeable equipment: 
                  Grouping “L” (Price Line 00018): requires an attached EXCLUSIVE N.J. NET PRICE LIST.

        Choose the group(s) that best suits the using agency and client needs. 
        Multiple Awards: the using agency chooses products from price lists. No one is designated as Primary for the grouping.  
         
        When choosing a group that has a Primary vendor designated: utilize Primary vendor first .   
        Secondary vendor is back-up, when Primary vendor cannot service the using agency.
