Confirmation of Eligibility
Recruitment into Sub-specialty Training 2022
This confirmation of eligibility form is for the sole purpose of confirming the candidates’ eligibility for
sub-specialty training. The form will be used at the longlisting stage to determine if any candidates are
ineligible at that stage, so completeness and accuracy are of vital importance. If there are any specific
concerns regarding the trainee’s application, please email ntn.grid@rcpch.ac.uk with further
information.
The confirmation of eligibility form should be completed by the Head of School (or
representative) and sent back completed to the candidate. Please complete one form per
applicant and include the sub-specialties they are applying to. If the applicant has
gained approval from the CSAC to count level 3 experience towards their training,
please complete a separate form for each sub-specialty they are applying to and
provide these details.
Head of School- please send signed copies of all eligibility forms completed for trainees in your
deanery to ntn.grid@rcpch.ac.uk
Applicants are required to submit the confirmation of eligibility form along with their completed Oriel
application by the application closing date 12 noon on Wednesday 17 November 2021. Any
applications submitted without the required confirmation of eligibility form will not be accepted.
Applicants therefore need to submit the form to their deanery contact by Monday 8 November 2021
at the latest, so that there is sufficient time for the deanery contact to complete and return the form to
the applicant in good time before the closing date.
Please note that Shape of Training will not affect the eligibility criteria for sub-specialty applicants
starting in post in 2022.
